Description:
This Request for Proposal is the second stage procurement process pursuant to University Health Network-s Request for Supplier Qualifications (“RFSQ”) Imaging Fleet Replacement No. 2021-25 and resulting Imaging Fleet Replacement Framework Agreement entered into by the Proponent and University Health Network (“UHN”). Proponents are invited to submit Proposals for the turnkey installation of one (1) Computed Tomography (“CT”) Scanner, for the Diagnostic Imaging dept at Thunder Bay Regional Health Sciences Centre (“TBRHSC”) 980 Oliver Road Thunder Bay, Ontario Canada. The successful proponent may have the opportunity to add one (1) additional CT Scanner, as further described with the RFP document, during the 2025-2026 fiscal year pending approved funding.

The following two documents are incorporated herein and form an integral part of this RFP:Request for Supplier Qualifications Imaging Fleet Replacement No. 2021-25 dated February 10, 2021 as issued by University Health Network.Imaging Fleet Replacement Framework Agreement as executed between University Health Network and the Proponent.**Note: Proponents must have a representative attend the mandatory site visit scheduled for Wednesday, June 19, 2024 at 1:00PM EDT (two (2) hour maximum duration) in order for their proposal to be considered for evaluation.**
