- This Request for Proposal (the “RFP”) is an invitation by the Regional Municipality of Niagara (“Niagara Region”) to prospective proponents to submit proposals for Seniors Services Foot Care Wellness Supportive Living Program in Niagara Regional Housing, as further described in Section A of the RFP Particulars (Appendix B) (the “Deliverables”).
The Wellness Supportive Living Program is a collaborative partnership of agency providers whom have mobilized a comprehensive wellness model of integrated approaches to planning and coordination to support seniors living in one of four designated Niagara Regional Housing Buildings. With a focus on enabling access to onsite services and recognition and early identification of high-risk needs, the program aims to build tenant capacity, reduce social isolation and increase access to services in an effort to reduce risk factors with a focus on upstream health promotion/prevention strategies. - Documents:
