Description : This is a Request for an Expression of Interest and Supplier Information to identify all Suppliers in the Canadian marketplace licensed to provide Mohawk Medbuy Members and Customers the following products: A virtual patient care platform with functionality to manage and rehabilitate patients remotely in their homes( also known as telehomecare), particularly those with chronic diseases such as Heart Failure and Chronic Obstructive Pulmonary Disease (COPD). Post-Operative Monitoring that manages patients discharged home after surgery, and Episodic Monitoring (i.e. managing COVID-19 patients isolated at home) may also be in scope. The objective is to improved quality of life, physical activity, and disease self-management, in addition to reduced risk of hospital readmission and emergency department visits. HL7 integration into common Canadian EHR systems will be required. New care tools need to be integrated within existing clinical workflows to ensure high end-high end-user adoption and to ensure electronic patient health data is captured efficiently in the patient-s chart.
