Description : PROGRAM EVALUATION STUDIES Section 1: OVERVIEW 1.1 Summary This is a request for proposals (RFP) to assist the New Brunswick Health Council (NBHC) in conducting the 2021-2022 New Brunswick Student Wellness Survey (SWS) using a mixed-method design. The scope of deliverables under this RFP includes: (1) designing an online survey and collecting responses, (2) designing paper questionnaires for scanning, (3) printing the questionnaires, (4) inserting the questionnaires in class envelopes, (5) sending the questionnaires to the schools, (6) capturing survey responses and (7) providing a database containing survey results. In addition to the main project described above, the NBHC is considering adding one or two of the following elements to the project.
