Detailed_Description : HMMS on behalf of Southwest Self Management (SWSM) is looking to establish a partnership with a preferred Proponent to build a comprehensive database to streamline data collection, tracking, and reporting processes. SWSM aims to enhance program efficiency by leveraging the new database and integrated Microsoft tools (e.g., Forms, Power Apps). These tools will streamline various aspects of the program, including data capture, program planning, workshop management, volunteer coordination, partner support, expense tracking, and reporting. Importantly, SWSM will continue to ensure that no personal health information (PHI) is collected from patients, volunteers, or partners.Ideal outcome: -General cost of solution - (if possible general breakout of costs) -Understanding of required timeframe for development -Understanding of training opportunities, and post-development support.
