Description : The Department of Health and Community Services invites proposals from interested community based service providers to provide short term residential care to individuals requiring access to timely rehabilitation and restorative care services following an acute episode or planned hospital stay prior to returning home. The goal of this service is to facilitate recovery and a return home, and to improve hospital flow. The project will be evaluated after one-year and the results of the evaluation will inform the development of one provincial program. Services will be offered up to eight weeks and are coordinated by the regional health authority (RHA) in collaboration with the client and the service provider. The RHA will help the client identify their needs, and develop a plan of care to help achieve their health goals. This community based option will reflect a philosophy of client centered care, where the dignity and autonomy of clients is respected and supported. Proponents must demonstrate that they have the required staffing, model of care philosophy, and infrastructure to provide a safe environment for clients.
