Description:
Detailed_Description : Ontario Health is seeking expressions of interest from practicing physicians in Ontario with extensive experience in integrated care initiatives for the position of Clinical Lead, Integrated Care. The Integrated Care Clinical Lead will provide expert advice and leadership to help inform the continued development and acceleration of Ontario Health Teams, as Ontario-s vehicle for integrated care. Specifically, the Integrated Care Clinical Lead will engage clinician leaders across the province and reflect the perspectives of clinical providers in informing policy and provincial implementation planning for OHTs. This individual will focus on advising Ontario Health on how to best support implementation of current clinical priorities for OHTs, inclusive of home care and chronic disease prevention and management. The Integrated Care Clinical Lead will report directly to the Vice President, Integrated Care. This opportunity is open to all qualified candidates.
